TSR1 Application Form
What is this form for?
- Use this form to apply online for registration in a recognised postgraduate training post in the trainee specialist division.
- This form can be used instead of submitting a hardcopy TSR1 form to the Medical Council.
- When you apply online, the Medical Council will obtain the details of your training post directly from the HSE.
- When you submit this form, you and the hospital you identify on the form will receive an automatic e-mail acknowledgement of your application.
Who can use this form?
Only currently-registered doctors can submit an online TSR1 application. If you are not currently registered, you should use the
TSR1 form.
The TSR1 form will also be available from the hospital where you have been offered a training post. The form maybe returned to the
Medical Council by fax or post.
IMPORTANT: Your details may be shared with relevant third parties, e.g. training bodies/HSE/external agent(s) acting on behalf of the Medical Council.
